Internal Medicine Associates of Auburn offers a Chronic Care Management program for their Medicare patients. The practice was one of the first to roll out this program.
The centers for Medicare & Medicaid identified the care of chronic health conditions as an important goal. The program requires that the patient is diagnosed with two or more chronic medical conditions to qualify for participation.
Many patients already benefit from this program as it represents a critical step forward in recognizing the essential features of primary care including a whole-person focus, and serving as the patients’ first contact for new health issues, and care coordination.
“Our goal is to offer high quality patient care coordination across the health care spectrum. We believe in treating the whole person, not just the disease. Our plan is to optimize our patient’s health, increase their quality of life and prevent hospitalization.”
